You are hereby notified that on March 27, 2017, a Preliminary Petition for Adoption of Jaidyn Marcus Taylor was filed in the Portage County Probate Court, Case Number 2017 AD 7. Said petition alleges that the father of Jaidyn Marcus is unknown.

A hearing to surrender your parental rights regarding Jaidyn Marcus Taylor and the placement of said child for adoption is scheduled for July 25, 2017 at 10:45 a.m. The court is located at 203 W. Main Street, Room 305, Ravenna, Ohio. You are hereby notified that failure to answer for any rights to said child or object to the placement of said adoption by the date set forth herein shall forever bar you from making any claims to said child, and any rights which you may otherwise have to said child shall thereafter be forever terminated.
